MICROSOFT WORD FORMATTED FORMS
If you would prefer to apply using the older Microsoft Word versions of the forms, If you do not want to complete an award application online, and would rather download the old-styled Microsoft Word formatted award application, then go THE NATIONAL AWARDS APPLICATION webpage, and underneath each individual award category is the option to download an MS Word formatted form. You must download that file, complete it inside of Microsoft Word, then email the completed application and attach any necessary documents to that same email.
DESCRIPTIONS OF THE AWARDS
Award descriptions can be found on the National Awards Application webpage.
PROCESS
Once award applications are received, and approved by Central Office, they are forwarded to the National Awards Chairman, Alexander B. Fafara, Jr. PNP. He then appointed seven three-man subcommittees (one for each award category) to review the applications. Once the decision of the subcommittee is made, the Awards Chairman is notified, and Central Office prepares the Award Plaque for the winner, for presentation at the National Awards Banquet to be held at the National Convention on Saturday, August 15, 2015. The names of the award winners are held in strict secrecy until that event.
Best of luck to all of our applicants and nominees.
